shivaya vishuNu rUpAya,
shiva rUpAya vishNave, shivascha hrudayaM vishNu, vishNoscha hrudayaM
shiva, yatha shivamayO vishNu, evam vishNumaya shivaH.

One also adds here
yathAntaram na pasyAmi
tathA me swstirAyushi.
