The above is as per the Kudali-Sringeri guruparampara , printed at the T.T.D. Press, Tirupati in 1986, and
I could see a copy of this with a friend in Mumbai. This also seems to match what the Dwarka matha says 
that Sri Sureshwaracharya was the first mathadhipati there. Sri Sureshwaracharya  could have installed his 
successor in Dwarka and left for Kudali Sringeri to relieve Sri Prithvidhara. Sri Sureshwaracharya could have
been simultaneously looking after the Kudali-Sringeri matha and the Kanchi Kamakoti matha.

 What I heard is that the Kanchi Kamakoti math thinks that
 Adi Shankaracharya established the Kudali-Shringeri math on
 the bank of the Tungabhadra and made Sri Prthvidhara alias
 Sri Hastamalaka (who hailed from Gokarna) as the first
 mathadhipati there. Later on Sri Sureshwaracharya took over
 the Kudali Sringeri math to releave Sri Prithvidhara, who
 wanted to go to Kanchi and eventually Sri Prithvidhara Swami
 had Samadhi there in Kanchipuram. May be you can recheck
 with your source in the Kanchi Kamakoti math.

  their travels, they reached Sringeri in Karnataka, which
 is
  on the banks of Tungabadra. While Sri Sankara and Mandana
  Mishra were walking, Sarasawani did not move and stood
 fixed
  in the sands of Tungabadra. Sri Sankara turned back and
  realised by his divine powers that Sarasawani did not want
  to proceed any further and created a seat for her for
  spreading the Advaita. This seat is today called the
 Sharada
  Peetham or the Seat of Sharada. This was the first Mutt
  installed by Sri Sankara, with the direction that all the
  heads of the Mutts will be called Sankaracharayas and they
